how seniors and their caregivers experience dementia support. Our
innovative GUIDE Program brings compassionate, comprehensive
dementia care directly into the home - empowering families, easing
caregiver burden, and improving quality of life. We are seeking a
dedicated and skilled Dementia Nurse Navigator to join our growing
interdisciplinary team. This role is ideal for an RN who is
passionate about dementia care, enjoys building meaningful
relationships with families, and thrives in a dynamic,
mission-driven environment. What You'll Do As a Dementia Nurse
Navigator, you will serve as a trusted partner to individuals
living with dementia and their caregivers. Key responsibilities
include: Conduct home safety assessments , identify risks, and
provide practical recommendations. Perform MoCA cognitive
assessments or coordinate alternative testing. Lead monthly
caregiver support calls using GUIDE-compliant questions.
Participate in interdisciplinary team (IDT) meetings and contribute
to care plan updates. Complete annual assessments and ensure timely
follow-up on safety concerns or care plan changes. Educate
caregivers on dementia progression, communication strategies,
safety, and symptom management . Provide accurate and timely
documentation in Athena. Assist families in accessing community
resources , adult day programs, respite options, and in-home
supports. Complete medication reconciliation and identify when
Advance Care Planning discussions are needed. Work Environment
Hybrid role: in-office, remote, and home visits required Flexible
scheduling within program needs Supportive, collaborative team
culture with training provided Qualifications Active Virginia RN
license Minimum 2 years of experience in dementia care, geriatrics,
home health, hospice, or primary care Knowledge of dementia
behaviors, caregiver stress, and safety strategies (Dementia
certifications preferred) Experience conducting cognitive
assessments (preferred) Strong communication, organization, and
documentation skills Comfortable working independently and managing
a dynamic caseload Proficiency with EHRs; Athena experience is a
plus Previous experience working with individuals living with
dementia ( additional training and certification provided ) Join a
